POSITION DESCRIPTION

 

Job Title:  Service Coordinator                                   Division/Dept:  Adult Services

 

Supervisor:  Program Coordinator

 

Positions Supervised:  Direct Support Staff/Skills Instructors

 

Summary Statement:

 

The Service Coordinator is responsible for planning, scheduling, staffing, coordination and monitoring services for participants at The Homestead Group.   Ensures that staff treats people with respect and listen carefully to them to best honor their choices, privacy, refusals and rituals while also providing appropriate safeguards.  Assists people to identify and resolve any needs or concerns they may have.  Assists people to learn about and advocate for their personal rights and to understand the responsibilities that accompany those rights.  Ensures that HIPAA regulations are met.  Ensures that staff supports people with the kind, amount and style of assistance they need to enjoy personal well being. Recognizes that people's need for support changes as their life circumstances change and adjust supports as needed.

 

Responsibilities or Duties:

 

1.      Develops and insures the implementation of Individualized Support Plans for program participants.  Develops goals, objectives, strategies, and guidelines based upon the needs of the individual, as determined by the Interdisciplinary Team.

 

2.      Coordinates, facilitates and implements the agreed upon services in a person’s Support Agreement, ISP and Staff Team Meetings and communicates these services effectively to all stake holders. 

 

3.      Develops and maintains an annual calendar of all scheduled responsibilities. 

 

4.      Provides and/or obtains counseling, guidance and education to program participants as necessary.  Coordinates all ancillary services and activities provided within the agency and in the community (psych, OT, PT, ST, etc.). 

 

5.      Maintains all case records according to professional standards, funding source guidelines and accrediting organizations. 

 

6.      Works with the MHRH DDD Social Worker, Human Rights Committee, Professional Review Process, family members and other involved parties/agencies. 

 

7.      Completes performance reviews for individuals under their direction.  Coaches and counsels employees regarding job performance as needed.  Provides personnel support services by interviewing, hiring and orienting new employees.  Monitors training for supported staff.

 

8.      Has a working knowledge of all jobs under his/her supervision.  Provides direct support services to include lifting and therapeutic holds as necessary.

 

9.      Facilitate and promote principles of self-advocacy which promote an independent lifestyle and respect for program participants. 

 

10.  Communicates effectively with a variety of community agencies, businesses and members, verbally or in writing. 

 

11.  Foster relationships with families, friends, etc. 

 

12.  Ensures all quality standards are met in compliance with required regulatory authorities. 

 

13.  Understands and practices principles of incident management process. 

 

14.  Ensures that medication and treatments are administered following proper medical procedure, monitors and reports medication errors and medical status as needed.

 

15.  Adheres to all Human Rights policies and procedures while providing the appropriate level of safeguards. 

 

16.  Follows doctors’ orders and ensures that medications and treatments are administered following proper medical procedures.  Monitors the dietary needs of the program participant.  Recognizes, reports and assists with medical crises as they occur.

 

17.  Ensures that all procedures for shift onset are followed. 

 

18.  Contributes to the positive morale by representing the agency through professional communication and interactions. 

 

19.  Arrives punctually, participates and actively contributes in all requires meetings, in-services, and trainings.  Relates information accurately and timely to other team members. 

 

20.  Follows all established policies and procedures and documents accordingly.

 

21.  Drives people to destinations necessary for them to experience a full and diversified life, following all safety and legal regulations.

 

22.  Responds effectively to potential safety hazards and emergencies.  Maintains a clean, orderly and safe environment by being respectful of property and equipment.  Follows proper maintenance instructions. 

 

23.  Able to work with minimal supervision.  Will be on-call.

 

24.  Provides support to other areas of the agency, as needed.

 

25.  Performs other duties as assigned.

 

 

Qualifications:

Education & Experience:

High school diploma or GED, three years of progressive experience in the human services field with at least one year of management experience preferably working with adults with developmental disabilities.  Associate’s or Bachelor’s Degree preferred. 

 

Licenses and/or Certifications:

Must have a valid driver’s license and be considered insurable by the agency’s insurance company.

Scope of Responsibility:  Provides direct supervision for a staff of approximately 10-20 people.
