What's Happening at THG

Take a FREE Personal Health Assessment online between July 1 – July 31
and enter to win one of four $25 gas cards.

The Personal Health Assessment (PHA) is a confidential questionnaire which takes roughly 15-20 minutes to complete, and asks participants a series of questions about their current health, medical history, beliefs and attitudes, and personal habits related to things like exercise, diet, automobile safety, and tobacco or alcohol use. Each PHA participant will receive a detailed report concerning their current health status and potential areas of risk, along with suggestions on how to achieve and maintain good health. Your responses and individual results will not be shared with your employer and will not affect your healthcare coverage or premiums in any way. To take your PHA, just follow these instructions.

  1. Go online to www.yourpha.com
  2. Click “New Users” and create a Username and Password.
  3. Your campaign code is:  Homestead8.  
  4. Answer all the questions in the assessment. Be sure to enter in the biometrics results after the clinic to receive a full report.
  5. Upon completion, you’ll receive your online PHA report.
